Terms of Service

These terms are a binding agreement between you and MXS Suppliments LLC, a Delaware limited liability company doing business as CoreScripts. Please read section 14 — it contains an arbitration agreement and a class-action waiver.

In plain language

CoreScripts is the technology and administrative company. It does not practice medicine and it does not dispense medication.

Completing an intake is free and does not buy you a prescription. A licensed clinician decides. If they decline, you are not charged.

If a clinician approves you, $199 recurs every month until you cancel. Cancel any time; medication that already shipped cannot be returned.

Disputes go to individual arbitration, not court and not a class action, unless you opt out within 30 days.

1Acceptance of these terms

These Terms of Service (the “Terms”) govern your access to and use of getcorescripts.com, the CoreScripts patient portal, and any related pages, applications, communications, and services (together, the “Services”). The Services are operated by MXS Suppliments LLC, a Delaware limited liability company with its principal place of business at 1309 Coffeen Ave, Ste 1200, Sheridan, WY 82801, doing business as CoreScripts (“CoreScripts,” “we,” “us,” or “our”).

By creating an account, submitting a medical intake, placing an order, or otherwise using the Services, you agree to these Terms and to our Privacy Policy, Telehealth Informed Consent, Refund & Cancellation Policy, and Shipping Policy, each incorporated here by reference. If you do not agree, do not use the Services.

2Eligibility

To use the Services you must:

  • Be 18 years of age or older;
  • Be physically located in the United States at the time of your consultation and at the time medication is shipped, in a state where our affiliated clinicians are licensed and where the Services are offered;
  • Have the legal capacity to enter into a binding contract; and
  • Provide accurate, complete, and current information about yourself and your health.

The Services are not directed to children. We do not knowingly collect information from anyone under 18. Availability varies by state and by pharmacy supply, and we may decline or discontinue service in any state at any time.

3What CoreScripts is, and what it is not

MXS Suppliments LLC d/b/a CoreScripts does not practice medicine. It is a technology and administrative services company. It does not provide medical advice, diagnosis, or treatment; it does not employ or control the professional judgment of any prescribing clinician; and it does not manufacture, dispense, or sell prescription medication.

Medical services are provided by CoreScripts Medical Group, P.A. and affiliated independent, physician-owned professional entities (the “Medical Group”). Clinicians of the Medical Group exercise independent professional judgment. Prescriptions, when issued, are dispensed by independent, state-licensed U.S. pharmacies, including CraftedRX.

Clinicians are compensated for their clinical time. They receive no additional payment for issuing a prescription, for the medication selected, or for the number of prescriptions written.

No prescription is guaranteed. Completing an intake, paying a fee, or creating an account does not entitle you to a prescription. A clinician may determine that treatment is not appropriate for you, may require additional information, laboratory testing, or a live visit, or may decline to prescribe for any clinically appropriate reason.

4Not for emergencies

The Services are not designed for emergencies and are not a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Do not use the portal, email, or messaging to report an emergency. Messages are not monitored continuously and response times are not guaranteed.

5Your account

You are responsible for the accuracy of the information you provide and for maintaining the confidentiality of your credentials. You agree to notify us promptly at support@getCoreScripts.com of any unauthorized use of your account.

Incomplete or inaccurate health information can lead to an unsafe prescribing decision. You agree to disclose your medical history, current medications, allergies, and relevant conditions truthfully and to update them if they change.

You may not share an account, order on behalf of another person, resell or redistribute any medication obtained through the Services, or use the Services to obtain medication for anyone other than yourself. Doing so may be a violation of state and federal law.

6Membership, pricing, and billing

The CoreScripts NAD+ program is a recurring monthly membership priced at $199 per month. That single price includes the clinical evaluation, the medication when prescribed, dose adjustments, secure messaging, and temperature-controlled shipping.

How billing works

  1. Today — $0. You provide payment details and we place an authorization hold. No funds are captured.
  2. Clinician review. A licensed clinician reviews your intake, usually within one business day.
  3. If approved — $199. Your payment method is charged $199 and your first shipment is released. The same amount recurs automatically every month on that calendar date until you cancel.
  4. If not approved — nothing. The authorization is released and you are not charged.

Authorization for recurring charges. By enrolling you authorize us and our payment processors to charge your payment method $199 per month, plus any applicable sales tax, on a recurring basis without further authorization, until you cancel. You authorize us to update stored card details through card-network account-updater services so that your membership is not interrupted.

Price changes. We may change the price with at least 30 days’ advance written notice to the email address on your account. The new price applies to renewals that occur after the notice period. If you do not agree, cancel before that renewal.

Taxes. Applicable state and local sales tax is added where required and is not included in the amounts stated above.

Failed payments. If a charge is declined we may retry it and may suspend or terminate your membership. Laboratory testing, if ordered by your clinician, is billed separately by the laboratory and is not included in the monthly price.

7Cancellation and refunds

You may cancel at any time. There is no fee and no minimum term. Cancel from Account → Plan in the portal, by emailing support@getCoreScripts.com, or by calling 1-888-888-8888.

Cancellation stops all future charges and shipments. To avoid the next charge, cancel before your renewal date.

Medication that has already been dispensed and shipped cannot be returned or refunded. Federal and state pharmacy law prohibits a pharmacy from accepting the return of a dispensed medication for resale or reuse. Full terms, including the limited circumstances in which we will make an exception, are in the Refund & Cancellation Policy.

8Telehealth and the clinical relationship

Care is delivered by telehealth. Before your first consultation you must review and accept the Telehealth Informed Consent, which describes the benefits, risks, and limitations of remote care.

You are matched only with a clinician holding an active, unrestricted license in the state where you are physically located at the time of the encounter. Your clinician’s name, credentials, and license number are available in your patient portal.

Your clinician’s decision is final on clinical matters. Neither CoreScripts nor any customer-service representative can override, appeal, or substitute for a clinician’s prescribing decision.

9Compounded medication disclosures

The NAD+ preparation offered through the Services is a compounded drug prepared by a state-licensed pharmacy against a patient-specific prescription.

  • Compounded drugs are not FDA-approved. The FDA does not review them for safety, effectiveness, or manufacturing quality before they are dispensed, and does not evaluate their labeling.
  • There is no FDA-approved injectable NAD+ product marketed in the United States.
  • NAD+ is not approved to prevent, treat, cure, or mitigate any disease, and we do not offer it as a treatment for one.
  • The long-term safety of injected NAD+ has not been established.

10Content, intellectual property, and license

All content on the Services — text, graphics, logos, images, layout, and software — is owned by MXS Suppliments LLC or its licensors and is protected by copyright, trademark, and other laws. CoreScripts and the CoreScripts wordmark are trade names and marks of MXS Suppliments LLC.

Subject to these Terms we grant you a limited, revocable, non-exclusive, non-transferable license to access the Services for your own personal, non-commercial use. You may not copy, scrape, frame, mirror, reverse-engineer, or create derivative works from the Services; use automated means to access them; or use them to build a competing product.

All third-party trademarks are the property of their respective owners. CoreScripts is not affiliated with, endorsed by, or sponsored by any pharmaceutical manufacturer, and reference to any medication does not imply it will be prescribed to you.

11Prohibited conduct

You agree not to: provide false information; impersonate another person; obtain medication for anyone other than yourself; resell, divert, or redistribute medication; interfere with or disrupt the Services or their security; attempt to gain unauthorized access to any account or system; upload malicious code; or use the Services for any unlawful purpose.

We may suspend or terminate your access, with or without notice, for conduct that violates these Terms, that we reasonably believe is unlawful, or that creates a risk to you, to other patients, or to us.

12Disclaimers

The informational content on the Services is provided for general informational purposes only and is not medical advice. It is not a substitute for consultation with a qualified healthcare provider. Never disregard professional medical advice or delay seeking it because of something you read on the Services.

EXCEPT AS EXPRESSLY STATED, THE SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, TIMELY, SECURE, OR ERROR-FREE.

This section does not limit or disclaim the professional obligations of any clinician of the Medical Group, and nothing in these Terms is intended to limit liability for professional negligence, gross negligence, willful misconduct, or fraud, or any liability that cannot be limited under applicable law. Some states do not allow certain exclusions, so parts of this section may not apply to you.

13Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, MXS SUPPLIMENTS LLC AND ITS OFFICERS, MEMBERS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, LOST DATA, OR LOSS OF GOODWILL, ARISING OUT OF OR RELATING TO THE SERVICES, WHETHER BASED IN CONTRACT, TORT, STRICT LIABILITY, OR ANY OTHER THEORY, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

TO THE MAXIMUM EXTENT PERMITTED BY LAW, OUR TOTAL AGGREGATE LIABILITY ARISING OUT OF OR RELATING TO THE SERVICES WILL NOT EXCEED THE GREATER OF (A) THE AMOUNTS YOU PAID US IN THE SIX (6) MONTHS PRECEDING THE EVENT GIVING RISE TO THE CLAIM, OR (B) ONE HUNDRED U.S. DOLLARS ($100).

You agree to indemnify and hold harmless MXS Suppliments LLC and its affiliates from any claim, loss, or expense (including reasonable attorneys’ fees) arising from your misuse of the Services, your violation of these Terms, or your provision of false or incomplete health information.

14Dispute resolution, arbitration, and class-action waiver

Please read this section carefully. It affects your legal rights.

Informal resolution first

Before starting arbitration, you agree to contact us at support@getCoreScripts.com with a written description of the dispute and the relief sought, and to allow 60 days for good-faith resolution.

Binding individual arbitration

Any dispute arising out of or relating to these Terms or the Services that is not resolved informally will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, rather than in court. The arbitration will be conducted by a single arbitrator, may proceed by written submissions or telephone unless the arbitrator orders otherwise, and any in-person hearing will be held in the federal judicial district of your residence. Judgment on the award may be entered in any court of competent jurisdiction.

Class-action waiver

YOU AND WE AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN AN INDIVIDUAL CAPACITY, AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY CLASS, COLLECTIVE, CONSOLIDATED, OR REPRESENTATIVE PROCEEDING. The arbitrator may not consolidate claims or preside over any form of representative proceeding.

Your right to opt out

You may opt out of this arbitration agreement by emailing support@getCoreScripts.com with the subject line “Arbitration Opt-Out” and your full name and account email within 30 days of first accepting these Terms. Opting out does not affect any other part of these Terms.

Exceptions

Either party may bring an individual claim in small-claims court, and either party may seek injunctive relief in court to protect intellectual property or account security. Nothing here waives any right you may have to file a complaint with a government agency or a state medical or pharmacy board.

15Governing law and venue

These Terms are governed by the laws of the State of Delaware, without regard to its conflict-of-laws rules, except that the Federal Arbitration Act governs section 14. For any dispute not subject to arbitration, you and we consent to the exclusive jurisdiction of the state and federal courts located in Delaware.

Nothing in this section deprives you of the protection of the mandatory consumer-protection laws of the state where you reside.

16Changes to these Terms

We may update these Terms. When we do, we will revise the “Last updated” date above and, for material changes, notify you by email or through the portal at least 30 days before the change takes effect for your membership. Continued use of the Services after the effective date constitutes acceptance. If you do not agree, cancel your membership before that date.

17General terms

Entire agreement. These Terms, together with the policies incorporated by reference, are the entire agreement between you and us regarding the Services.

Severability. If a provision is held unenforceable, it will be modified to the minimum extent necessary and the remainder will stay in effect.

No waiver. Our failure to enforce a provision is not a waiver of it.

Assignment. You may not assign these Terms. We may assign them to an affiliate or in connection with a merger, acquisition, or sale of assets.

Electronic communications. You consent to receive notices and disclosures electronically at the email address on your account.
